The Reentry Specialist facilitates access to community programs and ensures continuity of care for individuals transitioning from correctional facilities. They act as a liaison between various systems to support patients with mental health, substance use, and chronic medical illnesses.
Candidates must have a Bachelor's degree in Social Work, Psychology, or a related field and at least one year of experience in social services. Experience with Substance Use Disorders and case management is required, while certification in substance use counseling is strongly preferred.

The Reentry Specialist provides access to community programs, ensures continuity of care following release into the community, advocates for patient needs, and act as liaisons between the multiple systems impacting the lives of offenders with mental health illnesses, co-occurring substance use disorders, and chronic medical illnesses.
