The LPN provides high-quality nursing care to patients in their homes according to established plans of care and physician orders. They are responsible for monitoring patient conditions, documenting medication regimens, and collaborating with RN Case Managers to prevent avoidable hospitalizations.
Candidates must be a graduate of an accredited nursing program with a current State License as an LPN/LVN and at least one year of clinical experience. A valid driver's license, reliable transportation, and the ability to lift 50-100 lbs are required.

Responsibilities include: Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care Monitoring patient conditions and reporting significant changes to the RN Case Manager and healthcare team Assisting patients and caregivers with education and self-care techniques Implementing patient safety plans and utilizing available community resources Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes Documenting clinical findings, nursing interventions, and medication updates accurately and timely Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits Providing compassionate, professional support to patients, families, physicians, and colleagues
Required: Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program Current Massachusetts Licensed Practical Nurse (LPN) license Minimum of one (1) year of clinical nursing experience Valid driver's license, reliable transportation, and current auto insurance Ability and willingness to travel throughout the assigned territory Physical Requirements- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
