Provide direct physical therapy care to patients in their homes, including evaluating functional needs and implementing individualized plans of care. Collaborate with specialists and support staff to help patients achieve goals while maintaining accurate clinical documentation.
Must be a graduate of an APTA-approved Physical Therapy program with a current, unrestricted PT license and at least one year of healthcare experience. Requires a valid driver's license, reliable vehicle, and the physical ability to lift up to 100 lbs.
