• Job Summary Job Summary:The Nurse Practitioner (NP) specializing in house calls provides high-quality, patient-centered care to individuals in their homes, particularly focusing on those with chronic illnesses, mobility issues, or limited access to healthcare facilities.
• This role emphasizes holistic care, patient education, and the management of complex medical conditions.

Responsibilities

Responsibilities include: Patient Evaluations: Conduct comprehensive assessments of patients’ health status, including physical examinations, medical history reviews, and medication reconciliations during home visits. Diagnosis and Treatment: Diagnose medical conditions, develop and implement individualized treatment plans, and prescribe medications as needed. Care Coordination: Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care and comprehensive support for patients. Patient Education: Provide education to patients and their families regarding health management, lifestyle modifications, disease prevention, and medication adherence.
