Damar has a great opportunity for an Advanced Practice Provider - Women’s Health to be part of a multi-disciplinary team, in an integrated care setting that offers complete patient care, and the opportunity to collaborate with psychiatrists, clinicians, physicians, nursing staff, and community health workers. The Advanced Practice Provider – Women’s Health is responsible for providing patient-centered care in an outpatient setting, including prenatal assessments, postpartum care, routine women’s health services, and patient education. This role collaborates with physicians and interdisciplinary care teams to deliver high-quality, evidence-based care. The Advanced Practice Provider supports Damar’s patient population by delivering safe, effective services while addressing medical, behavioral health, and social needs. Through education, advocacy, and collaboration, this role helps improve health outcomes and empowers patients throughout their care journey. Where would I be working? Provides prenatal, postpartum, and routine women’s health services in an outpatient setting within scope of practice
Conducts comprehensive patient assessments, including medical histories, physical exams, and risk evaluations
Develops and manages individualized care plans in collaboration with patients and the interdisciplinary care team
Monitors maternal health and fetal development, identifying and addressing any complications or risk factors
Provides education and counseling on pregnancy, postpartum care, breastfeeding, and family planning
Promotes positive health behaviors and supports patients in making informed decisions about their care
Collaborates with physicians, nurses, behavioral health providers, and internal and external medical providers to ensure coordinated, integrated, and continuous care
Identifies and responds to behavioral health needs, social determinants of health, and barriers to care, connecting patients with appropriate resources
Facilitates referrals to specialists or higher levels of care when clinically appropriate
Ensures patient documentation is accurate, timely, and compliant with HIPAA and regulatory standards
Maintains accurate, complete, and timely documentation in accordance with organizational, state, and federal requirementsParticipates in quality improvement initiatives focused on maternal health outcomes and reducing health disparities
Provides culturally responsive, trauma-informed, and patient-centered care to a diverse population, including individuals with intellectual and developmental disabilities
Ensures all activities, services, and programs through Damar are conducted in a manner that is sensitive to and shows respect for the cultural and ethnic diversity for all Damar patients
Utilizes resources, materials, equipment, and staff effectively to maximize efficiency and quality of care
Serves as a member of the interdisciplinary team and exchanges clinical and medical information with internal and contracted providers
Maintains on-call availability and responds to patient needs as required
Adheres to all Damar policies, procedures, and regulatory requirements, including the Employee Handbook and operational policies

Master’s degree in Nursing (Nurse Practitioner) or completion of an accredited Physician Assistant program required Experience in women’s health, obstetrics, or prenatal care preferred Prior labor and delivery or obstetrics experience strongly preferred Current and active Indiana license as a Nurse Practitioner (NP) or Physician Assistant (PA) required Women’s Health Nurse Practitioner board certification (WHNP-BC) required for Nurse Practitioners Certificate of Added Qualifications (CAQ) in Obstetrics and Gynecology required for Physician Assistants DEA license or ability to obtain prior to start required CPR certification required Trauma informed care and de-escalation training Minimum age of 21 per licensure requirements Must meet and maintain background screening requirements for all Damar Programs as detailed in policy 1.HR.02 Pre-Employment and Employee Background Check Valid Indiana driver’s license and driving record that meets eligibility requirements of Damar’s insurance carrier (for all positions requiring driving) Successful completion of pre-employment physical required
