Oversee and manage the comprehensive end-of-life care for hospice patients, including adjusting care plans and administering medications. Coordinate with an interdisciplinary team to provide compassionate support and education to both patients and their families.
Must be a Registered Nurse with a current state license and a valid driver's license with auto insurance. One year of acute care experience is preferred, and hospice experience is desired.

HOW YOU'LL WORK: You'll oversee the overall care of the patient from beginning to end of life. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state. MAJOR AREAS OF RESPONSIBILITY: Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and make adjustments as needs change. Minimize Patient Risk: Update all doctor orders in patient chart and administer medication as prescribed by the physician. Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges. Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
Registered Nurse with current license in the state of employment. Minimum one-year experience as an RN in an acute care setting is preferred. Hospice experience is desired.
