Provide one-on-one home care to clients with illnesses or chronic diseases, assisting with daily living activities and hygiene. Monitor patient vitals and glucose levels while collaborating with a nursing team to follow care plans.
Must have a high school diploma, an active Illinois Nursing Assistant Certification, and CPR certification. A valid driver's license, auto insurance, and the ability to lift up to 50 lbs are required.

7 days per week 7a-5p shift in Joliet
2-3 shifts per week 11p-7a in Manhattan
