The Social Worker provides emotional and psychosocial support to residents and families while managing discharge planning and care coordination. They collaborate with an interdisciplinary team to develop individualized care plans and act as a liaison between residents and outside agencies.
Requirements summary
A Master's Degree in Social Work or a related human services field and a current CSW or LSW license are required. Preference is given to candidates with at least one year of experience in long-term care or skilled nursing environments.

Salary Range
$55,000 – $70,000 (Full-Time) Practice Settings:

Essential Duties and Responsibilities 1.
Participates in an interdisciplinary team approach to support resident care, discharge planning, and overall quality outcomes.

Provides therapeutic intervention and emotional support to residents and families coping with illness, disability, institutionalization, or life changes.
3.
Completes psychosocial assessments and maintains timely, accurate documentation in accordance with company policy and regulatory requirements.
4.
Develops and implements individualized social service care plans with measurable goals and interventions.
5.
Participates in interdisciplinary care plan meetings and ensures resident and family involvement in the care planning process.
6.
Acts as a liaison between residents, families, facility leadership, and outside agencies to support resident rights and advocacy.
7.
Conducts family meetings, support meetings, and other resident-centered communication initiatives as needed.

Facilitates discharge planning and coordination of services to support safe and successful transitions of care.

Assists with admissions processes including referral coordination, insurance verification, and facility tours when needed.
10.
Develops and maintains positive relationships with referral sources, healthcare partners, and community organizations.
11.
Supports a safe environment for residents, visitors, and staff while following all center policies and procedures.
Position Requirements
Master’s Degree in Social Work or related human services field.
Current CSW or LSW licensure within the state of operations.
Minimum of one year of Social Work experience preferred.
Long-term care or skilled nursing experience preferred.
Strong communication, organizational, and interpersonal skills.
Ability to work collaboratively within an interdisciplinary healthcare environment.
