This position primarily manages patient care by promoting effective education, self-management support, and timely health care delivery. Core duties involve serving as a liaison between residents, physicians, and families regarding health issues and ensuring regulatory compliance.
To perform this job successfully, the individual must satisfactorily perform all essential duties, which include managing various aspects of patient care and coordinating continuity of care. Qualifications require either a Bachelor's degree or one to two years of related experience and/or training, or an equivalent combination.
This position is primarily responsible for managing the care of patients, promoting effective education, self-management support, and timely health care delivery by performing the following duties.
This job has no supervisory responsibilities.
