Requires an Associate or Bachelor's degree in a Human Services field. Must possess a valid driver's license, reliable transportation with insurance, and strong communication skills.

EBCAP is seeking a full-time Case Manager (Community Treatment Specialist) to join our multi-disciplinary Assertive Community Treatment (ACT) Team. This rewarding position offers the opportunity to provide essential support services to clients in our community.
Key Responsibilities Perform comprehensive case management and care coordination duties on our Community Support Team Provide direct assistance, advocacy, coordination of care, and individualized support to clients Assist clients in scheduling and attending medical, eye, and dental health care appointments Document all encounters in the electronic medical record within 48 hours Complete required clinical documentation including Treatment Plans and Yearly Assessments to meet licensing and regulatory standards Meet established productivity standards Help clients access state and federal entitlements including SSI, SSDI, food stamps, and Medicaid Assist clients in obtaining and maintaining safe and affordable housing Schedule
This is a full-time position, 40 hours per week.
Associate or Bachelor's Degree in Human Services field Valid driver's license, acceptable driving record, and reliable transportation with insurance (travel required) Strong interpersonal and communication skills Ability to work independently and as part of a collaborative team
