Must be a graduate of an accredited Physical Therapy program with a current active license in the state of Oklahoma. Previous experience in long-term care or skilled nursing settings is preferred.
Bradbury Commons is seeking a compassionate and motivated Full-Time Physical Therapist to join our interdisciplinary care team in a long-term care setting. This role is focused on helping residents maintain or improve mobility, function, and overall quality of life through individualized, evidence-based therapy programs. The ideal candidate is patient-centered, organized, and comfortable working with geriatric populations, including those with chronic conditions, post-surgical needs, or mobility limitations. Key Responsibilities Conduct comprehensive evaluations of residents to assess physical function, mobility, strength, balance, and endurance Develop individualized treatment plans based on clinical findings, physician orders, and resident goals Provide skilled physical therapy interventions, including therapeutic exercise, gait training, transfer training, balance activities, and pain management techniques Monitor resident progress and modify treatment plans as needed to ensure optimal outcomes Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other team members to coordinate care Educate residents and families on treatment plans, safety strategies, and home exercise programs Ensure all documentation is accurate, timely, and compliant with state, federal, and facility guidelines Participate in care planning meetings and contribute to interdisciplinary team discussions Maintain a safe and clean therapy environment and adhere to infection control standards Support facility goals related to quality outcomes, resident satisfaction, and regulatory compliance
