As an RN or LPN in our Extended Care Department, you'll play a vital role in delivering patient-centered nursing care while building meaningful relationships with residents and their families. What You'll Do Provide safe, compassionate, and comprehensive nursing care to extended care residents Supervise and support non-professional staff; provide guidance and instruction as needed Adhere to nursing standards, policies, and procedures to ensure high-quality care Participate in unit conferences and the interdisciplinary team (IDT) process, contributing to continuous improvement in resident care Complete thorough nursing assessments, including health histories and ongoing evaluations Identify changes in resident condition, interpret diagnostic data, and communicate findings to providers and care team members Contribute to and implement individualized care plans in collaboration with residents, families, and the healthcare team Document resident responses to care accurately in medical records Maintain accountability for assigned residents and demonstrate leadership in daily practice Support professional growth through continuing education and ongoing learning
What We're Looking For Active Montana RN or LPN license (or compact license with multistate privileges) BLS certification required ACLS, Geriatric Nursing, Hospice Certification, or CHPN preferred (may be obtained after hire) Strong communication, time management, and teamwork skills Previous nursing experience preferred; new graduates are encouraged to apply Flexibility to work rotating weekends, holidays, and occasional on-call shifts
Physical Requirements Ability to ambulate independently and respond quickly in emergency situations Ability to perform CPR Frequent standing, walking, bending, reaching, and repetitive tasks Ability to lift, move, and reposition patients up to 50 lbs. using proper equipment and techniques Ability to perform essential job functions with or without reasonable accommodation
