Coordinates integrated care and comprehensive discharge planning for patients across inpatient and outpatient settings. Collaborates with clinical teams to assess risk factors, provide psychosocial support, and ensure patients have access to necessary medical equipment and community resources.
Requires a Master's degree in Social Work, Mental Health, Psychology, or Marriage and Family Therapy. Candidates need two years of direct clinical experience, though a Master's level internship may substitute, and HWC certification is required for the Behavioral Health Unit.
Collaborates with the assigned clinical team to identify patients most likely to benefit from care coordination services to include assessing patients’ risk factors and the need for care coordination, clinical utilization management and preventative care services.

Two (2) years of direct clinical experience with an emphasis on the population to be served in the assigned area. Successful completion of Master’s level internship within the population to be served may substitute the two (2) years of experience. Medical case management within an acute hospital setting highly preferred/a plus.
