Provide compassionate spiritual and emotional support to end-of-life patients and their families. Develop individualized spiritual care plans and collaborate with an interdisciplinary team to ensure holistic patient well-being.
Requires at least one year of experience as a Chaplain, preferably in a healthcare or hospice setting. A valid driver's license and reliable transportation are mandatory, and Clinical Pastoral Education (CPE) is preferred.

Assess and address the bereavement needs of patients, families, and caregivers with sensitivity and care. Develop, implement, and regularly update individualized, goal-oriented spiritual plans of care that reflect each patient’s unique beliefs, values, and preferences. Employ a range of counseling and spiritual care techniques to help patients and families find comfort, meaning, and peace. Accurately document all assessments, interventions, and communications in the electronic medical record in accordance with agency policies and professional standards Collaborate closely with the interdisciplinary hospice team to ensure coordinated, holistic care that supports the physical, emotional, and spiritual well-being of each patient and family. What You’ll Bring: One unit of Clinical Pastoral Education (CPE) or its equivalent is preferred. Minimum of one year of experience as a Chaplain, preferably in a health care/Hospice setting.
