The Case Manager implements the SBIRT public health model to coordinate wraparound services for adolescents and young adults. Responsibilities include maintaining contact with families, managing patient data, and conducting grant-required surveys.
Candidates need a Bachelor's degree in a related field, or an Associate's degree with 3 years of behavioral health experience, or a high school diploma with 6 years of experience. Ability to travel locally and perform light physical duties is required.

This position will be located at 699 Hertel Ave, Buffalo NY. Local travel may be required as needed.
This is a full-time position from Monday - Friday.
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $23.45 - $25.40. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
This is a grant-funded position.
