Geriatric Social Worker Responsibilities & Duties Assess the psychosocial needs of elderly clients Develop and implement individualized care plans Advocate for the rights and needs of seniors Provide counseling and emotional support to clients and their families Coordinate services such as healthcare, housing, and legal assistance Facilitate support groups for seniors and their caregivers Collaborate with medical professionals and other service providers Educate clients and families on aging-related issues Assist with end-of-life planning and decision-making Maintain client records and documentation Evaluate the effectiveness of care plans and adjust as needed Navigate clients through Medicare, Medicaid, and other benefit programs Promote social and recreational activities for seniors Requirements: Master's degree required.
Valid license as an LCSW issued by the NYSED.
Experience with assigned patient population and Utilization of PM Schedule and CWS.
Experience treating concurring substance abuse and mental illness required.
Membership in the NASW Academy is a plus.
