06/18/2026 Total Compensation Estimator ToolOpen Competitive Your Specific Responsibilities: The Linkage to Care Coordinator (LTC)/Disease intervention specialist (DIS) representative is a regional position within the Area 15 and is responsible for identifying, linking newly and/or re-engaging previously diagnosed HIV positive individuals into HIV/AIDS system of care with Area 15 (St Lucie, Martin, Okeechobee, Indian River County). This position is also expected to manage non-compliant and/or potentially non-compliant HIV positive clients. The incumbent is also expected to conduct counseling and testing in the community and in clinic settings in Area 15. The incumbent is responsible for assisting linkage/re-engagement activities and referrals for the area by ensuring individuals are linked to care within 30 days and that linkage to care field records in the HIV/AIDS linkage to care module and STARS are followed up on and closed out within timely manner. This position will assist with Data-to-Care linkage records and assure linkages and referrals to medical, social services and treatment services. Must be trained in phlebotomy and Disease Intervention Specialist. Works directly under the Supervision of Operations and Management Consultant II. Must be willing to travel out of the county and work before and after hours, including weekends. Identify newly diagnosed clients, re-engage clients who have fallen out of care, and assist providers, in locating non-compliant clients and bring them back into care and treatment. Ensure all newly diagnosed individuals are linked to care within 30 days. Ensures previously diagnosed and re-engaged into care within timelines, coordinate with local Regional Linkage to Care Coordinator (RLC), HIV/AIDS surveillance and STD program staff to ensure Data to Care (D2C), Molecular Monitoring Project (MMP), Molecular HIV surveillance (MHS), and Ryan White Re-engagement line list are worked in a timely manner and complete the D2C collection form to appropriate staff. The incumbent is responsible for identifying barriers to care and assist HIV positive individuals with necessary referrals and/or practical aspects of adherence such as transportation, obtaining needed medications, receiving Medicaid or private insurance, etc. Consults with public or private personnel, epidemiologists, ARNPS, infection control nurses, medical laboratory technologists, Technicians personnel and other health care providers to: (1) Stimulate case reporting, (2) Obtain information on treated cases or diagnostic determination. (3) Secure permission to apply epidemiologic process to private patients, and (4) Stimulate and educate providers by influencing the State Department of Health recommendations. Communicate regularly with RLC, HIV surveillance, STDs and other staff to assist with linkage and re-engagement activities for the area/county. Complete necessary referrals. Obtains pertinent information from medical providers or facilities regarding client care. Complete fields investigations, record searches, phone calls, and other contact attempts and enter all testing, linkage and/or re-engagement services into the Health Management System (HMS), the State Ryan White CAREWare system, STARS, Linkage To Care Module system and any other reporting systems as needed on a regular basis. Performs necessary case management activities to ensure clients stay compliant to care including but not limited to work prioritization, records documentation and management, clustering, HIV/STD testing and counseling, Ryan White Eligibility, Test and Treat linkage. Provides interview and/or re-interviews partner services for clients when DIS are unable to reach/or are unable to receive partners from, offers HIV/STD testing to other individuals that are at risk, utilizes all record searching software and electronic medical record with proper confidentiality and security, educates patients on disease concepts and referrals programs, communicates with public and private clinics, clinicians, hospitals laboratories and other health care providers to obtain demographic, medical or other specific client information and including test results, diagnosis, treatment, etc. and/or to arrange medical evaluations and treatments for a client. Attempt to have all clients enrolled in care and attended first appointment by 14 days of diagnosis. Develops appropriate materials, resources, and processes to improve the outcome and efficiency of quality care for HIV positive clients, high-risk clients, CBOs, and LTC staff. Participates in and disseminates information on trainings increase efficiency of program. Discuss each case with supervisor or designee prior to closing LTC record. Participates in community, or impromptu screening activities by performing HIV/STDs/Hepatitis venipuncture and/or using other rapid testing techniques. Collaborates with other DOH or community partners to provide outreach counseling and testing education/presentations at all community events including detention facilities and/or in DOH clinics. Ensure complete and accurate processing of specimens and data collected. May act as an outreach lead. Prepares and maintain timely and accurate records & reports according to program guidelines and requirements. Responsible for fostering and initiating a client led support group for those clients wishing to participate in a peer led monthly activity and assist with recruitment. Utilize collaborative agencies for buy in, adverstising and assistance. Attend monthly community/staff meeting. Provide feedback to supervisor and providers through debriefing sessions. Develops appropriate materials, resources, and processes to improve the outcome and efficiency of quality care for HIV positive clients, high-risk clients, CBOs, and LTC staff. Participates in and disseminates information on trainings increase efficiency of program. Incumbent is expected to serves as back-up to the Jail Linkage Coordinator: A) Performs direct HIV/STD/Hepatitis Counseling and Testing Services with the current preferred method (finger stick for rapid and venipuncture (blood draw) for confirmatory test on all positives) on inmates submitting and consenting to have such tests performed in accordance with Florida Department of Health HIV/STD Testing Guidelines and processes specimen collections for submission to the State Laboratory. Performs direct Court Ordered HIV/STD counseling and testing services when required. B) Provides medical and other ancillary services within the detention facility for the purpose of coordinating services and ensuring that Jail Linkage Programmatic Services needs and goals are provided at the highest level of quality and accessible to all inmates where applicable. C) Facilitates regular HIV/STD Prevention education classes for inmates of the detention facility and distributes and collects facilitator evaluation forms and knowledge-based HIV/STD pre and post tests at the completion of each class. D) Identify confirmatory HIV/STD Positive Test results performs original posttest counseling interviews and Partner Counseling, testing and Referral Services, and links individuals testing positive into the care system both internally within the detention facility and externally upon release in collaboration with the Linkage to Care Program. Maintains client confidentiality and security in accordance DOHP50-10 Information Security & Privacy Policy Confidential Information. Records, certifies and submits EARs, Cell Phone verification, Everbridge Alerts, CSRs and timesheets per DOH policy within specified policy time frames. Participate in department and unit meetings. Performs other related duties as assigned or when required. Ensures the accuracy and completion of HIV/STD counseling and testing forms per state guidelines. Submit logs, final list of responsive external providers, businesses, appropriate organizations/venues and monthly monitoring reports to immediate supervisor. Participate in local/ statewide, meetings, training and local committee, staff, and unit meetings. Required Knowledge, Skills, and Abilities: Knowledge, skills and abilities, including utilization of equipment, required for the position: Must be able to travel in and out of county within area 15, must a valid DL, case manager experience, phlebotomy training. Self-motivated, self-starter willing to take initiative, flexible, ability in PowerPoint presentations skills, exceptional interpersonal skills, efficient in communication, good organizational and time management skills, ability to work in teams, Able to complete Linkage to Care (ARTAS) and Disease Intervention Specialist training, experience with delegating tasks, must be comfortable with talking to individuals and groups. Knowledge and experience in working with diverse communities. Ability to plan and coordinate work assignments. Knowledge of Microsoft Office (Word, PowerPoint, Excel, Outlook).
Minimum: Must be willing to become Certified in Phlebotomy and complete training on Disease Intervention Specialist and Linkage to Care ARTAS training. Must be willing to travel out of county, undesirable neighborhoods and/or between counties Must be willing to work before and after hours and during weekends Must be willing to serve in special needs shelter during hurricanes and/or any other emergency duties involving natural and/or man-made emergency Must have a minimum 1- year case management experience and/or patient care coordination/navigation experience Must be willing to provide services to all residents in detention facilities/jails, rehabilitation/drug addiction facilities, mental health facilities, and/or subcontractors in Area 15. Must have a valid driver’s license Must pass an additional required background screening conducted Must be able to lift/move up to 50 pounds, bend, stoop, reach, and climb a ladder.
