Must hold a current LPN license in Tennessee, CPR certification, and a valid driver's license with auto insurance. Previous home health experience is preferred but not required.
Licensed Practical Nurse | PRN | | Nashville, TN All Heart Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our home health team in the Nashville area. This PRN position offers flexible, as-needed scheduling while providing personalized, high-quality care to patients in their homes. Key Responsibilities: Provide skilled nursing care to patients in accordance with the plan of care and physician orders Monitor patient health status and report changes to the supervising Registered Nurse or physician Administer medications and treatments as prescribed Assist with wound care, vital signs monitoring, and other clinical tasks Educate patients and caregivers on disease management, medications, and home care techniques Maintain accurate and timely documentation in compliance with company policies and regulatory standards Collaborate with the interdisciplinary care team to ensure coordinated patient care Travel to patient homes within the assigned service area
