Travel between multiple clinics to support outpatient vascular, wound care, and dialysis access evaluations. Responsibilities include performing wound assessments, dressing changes, and monitoring patients with peripheral arterial disease.
Must hold a current Texas LVN license, a valid driver's license, and have reliable transportation for regular travel. BLS certification is preferred, and experience in wound care or dialysis access is desired.
Travel to assigned clinic locations in San Antonio, Hondo, Lytle, Boerne, and Eagle Pass. Collect health history and documents accurately in the EMR. Assist with evaluation and monitoring of patients with PAD, lower-extremity wounds, foot ulcerations, and related vascular concerns. Support evaluation of dialysis vascular access, including tunneled dialysis catheters (permcaths), AV fistulas, and AV grafts. Perform wound assessments, dressing changes, patient education, and follow-up care under provider direction as needed. Monitor and report changes in wound status, circulation concerns, dialysis access concerns, drainage, swelling, pain, or signs of infection.
Current Texas LVN license in good standing. Valid Texas driver's license in good standing. Must be a covered driver on vehicle insurance. Must have reliable transportation. Ability and willingness to travel regularly between clinic locations. BLS certification preferred or required upon hire. Experience in wound care, vascular, dialysis access, podiatry, family practice, or outpatient clinic settings preferred. Strong communication, documentation, and organizational skills.
$5,000 sign-on bonus paid over 6 months. Mileage reimbursement, car allowance, or gas stipend. Paid time off Medical, dental, vision, life insurance, STD, and LTD offered 401K eligibility after 1 year. Paid scrubs.
