The Social Worker promotes patient and family wellness and manages comprehensive transitional care plans. They facilitate access to hospital and community resources for patients with complex psychosocial needs in inpatient and emergency settings.
A Master's degree from an accredited social work program and 2 years of experience in an acute or post-acute setting are required. A Licensed Medical Social Worker (LMSW) license in Georgia is preferred.
The Social Worker strives to promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources, and manages, supports and develops comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments.
