Perform home health visits to evaluate clients' functional status and develop individualized rehabilitative intervention plans. Educate clients and families on safety and independent living while documenting all care management services.
Must be a graduate of an accredited Occupational Therapy program with a valid Minnesota OT license. Requires a valid driver's license, reliable vehicle, and the ability to work flexible hours.

Our OTs frequently travel throughout either the Twin Cities metro area or St. Cloud metro area from one client’s home to another.
