Conduct initial home health care OASIS assessments and develop comprehensive care plans for patients. Supervise LPNs, CNAs, and HHAs while coordinating with interdisciplinary teams to ensure quality care.
Requires a valid Registered Nurse license in Minnesota and graduation from an accredited nursing program, with a BSN preferred. Must have at least one year of relevant home care or assisted living experience and a valid driver's license with a reliable vehicle.

At CareAparent you will work with other passionate individuals focused on giving the best care possible for our clients. The Start of Care/Resumption of Care Registered Nurse (SOC/ROC RN) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising HHA's and CNAs. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Our Case Management model ensures you will receive timely training and support to help you provide the best care possible.
$32.00 - $45.00 hourly + Millage reimbursement at Federal rate
