The Physical Therapist provides direct care and therapy services to patients in their homes, including comprehensive evaluations and the development of care plans. They are responsible for supervising Physical Therapy Assistants and Home Health Aides while maintaining detailed documentation in the EMR.
Candidates must have a Bachelor of Science in Physical Therapy and a valid Connecticut license, with a master's or doctorate preferred. A minimum of two years of clinical experience in acute or sub-acute care is required, along with CPR certification and a valid driver's license.
