Develop and implement nutritional plans of care based on comprehensive patient assessments. Provide dietary education and counseling to patients and caregivers within a home setting as part of an interdisciplinary team.
Must be an Indiana Certified or Registered Dietitian with a Master's degree in dietitian studies. Requires a valid driver's license, reliable automobile, and the ability to pass background and drug screenings.
