Provide care coordination and crisis intervention for youth and adolescents with severe emotional and behavioral needs. Coordinate with providers and families to implement holistic, family-driven plans following the High Fidelity Wraparound process.
Requires a Bachelor's degree in Social Work or a related field with 2 years of experience, or a Master's degree with 1 year of experience. Must be bilingual in English and Spanish and possess a valid driver's license and vehicle.

We provide state-of-the-art programs and services and compassionate care to more than 20,000 people of all ages and backgrounds each year.
We are seeking a full-time bilingual (English/Spanish) Care Manager to join our innovative High Fidelity Wraparound (HFW) team. HFW is an evidence-based process that supports youth with severe emotional and behavioral needs. HFW provides team-based planning, that is comprehensive, well-coordinated, holistic and family/youth-driven, with the intent to meet the complex needs of youth with serious mental health needs. The goal is to bring all parties to the planning table, to support a plan that is responsive to the family’s identified needs.
The salary for this full-time position is $55,000 - $60,000, and provides extensive training.
