The APP provides autonomous clinical care in long-term care facilities to improve resident stability and reduce hospitalizations. Responsibilities include managing acute and chronic conditions, developing care plans, and supporting regulatory documentation.
Requires an advanced degree (MSN, DNP, MPAS, or DMSc) with active state licensure and board certification. Candidates must hold DEA and BLS certifications, with a preference for 2+ years of geriatric or long-term care experience.

MSN, DNP, MPAS, DMSc, or equivalent advanced degree. Active state licensure and board certification (AANP/ANCC/NCCPA). DEA and BLS certification. Strong interpersonal communication and knowledge of clinical practice standards.
It’s A Plus If You Have 2+ years of clinical experience. Long Term Care, Skilled Nursing Facility, or geriatric experience.
