Provide comprehensive primary care services for adult and pediatric populations, including diagnosis, treatment, and preventive health maintenance. Collaborate with a multidisciplinary medical team to deliver integrated, patient-centered care and meet quality performance metrics.
Must be a graduate of an accredited Physician Assistant program with a current Colorado license, prescriptive authority, and BLS/CPR certification. A minimum of 3 years of clinical experience and DOT certification are preferred.
As a Physician Assistant, you will play a central role in delivering integrated, patient-centered care and APM performance metrics. Under the supervision of the Manager and Senior Vice President of Medical Services, the essential functions and other duties include: Clinical Care Provide comprehensive primary care across the lifespan, including health assessments, physical exams, diagnostic evaluations, and management of acute and chronic conditions using evidence-based guidelines. Order, perform, and interpret diagnostic tests (e.g., blood work, imaging, EKGs, urinalysis) to support accurate diagnoses. Develop and adjust individualized treatment plans, prescribing medications, therapies, and preventive interventions as appropriate. Deliver preventive services such as wellness visits, vaccinations, screenings, and routine check-ups. Engage patients in shared decision-making and self-management support; educate on disease prevention, healthy habits, and self-care strategies. Prepare and administer prescribed medications, injections, and vaccinations. Evaluate behavioral health assessments and refer to behavioral health professionals as needed. Perform office-based procedures within scope of practice and participate in acute care situations (e.g., urgent triage). Care Integration & Collaboration Participate in daily huddles, case conferences, and care planning meetings. Coordinate referrals and care transitions across multiple providers and healthcare settings. Quality & Compliance Maintain accurate, timely electronic health records and documentation supporting medical necessity, quality metrics, and compliant billing; collaborate with billing and quality teams as needed. Deliver whole-person orientation, coordinated care, enhanced access, and continuous quality improvement. Participate in quality initiatives and use data to drive performance on APM measures (e.g., patient engagement, preventive screenings, chronic disease outcomes). Meet productivity, patient access, and panel management benchmarks aligned with organizational and payer requirements. Adhere to confidentiality standards, medical ethics, and Colorado statutes related to collaborative practice and prescriptive authority. Professional Development Stay current with medical research and guidelines through continuing education and integration of evidence-based practices. Advance the education of healthcare team members to promote high-quality care. Physical Requirements: Must be able to remain on feet for prolonged periods while evaluating and treating patients. Must have adequate vision (with or without correction) to read medical records, observe patient conditions, and perform diagnostic procedures. Must be able to work in environments with potential exposure to communicable diseases, bloodborne pathogens, and other hazardous substances, while adhering to appropriate safety protocols and use of personal protective equipment (PPE). Must be able to move rapidly and react appropriately in urgent or high-stress clinical scenarios. Current, unrestricted Physician Assistant license in the State of Colorado. Prescriptive Authority recognized by the Colorado Medical Board. Active DEA registration (or the ability to obtain). Current Basic Life Support/Cardiopulmonary Resuscitation Certification. Must be insurable under company-provided malpractice coverage. Computer proficiency.
