The Wound Care Clinician performs skilled nursing assessments and specialized care for patients with actual or at-risk skin integrity deficits, including pressure ulcers and various types of wounds. Key duties involve case finding, consulting, formulating treatment plans, educating staff and physicians, auditing charts, and managing quality improvement processes.
Candidates must possess a Bachelor's Degree and be currently licensed in Connecticut as a Registered Professional Nurse, along with a minimum of five years of experience in an acute care setting. Desired qualification includes certification by the Wound Ostomy Continence Society (WOCN) as a Wound Care Nurse, or achieving this certification within 12 months of hire.
Monday thru Friday 0700-1530. Off shift if needed.

The Wound Care Clinician performs skilled nursing assessment and specialized care, including interventions, treatment and education to patients at risk for or with actual deficits in skin integrity, such as those with pressure ulcers, arterial, venous, diabetic and other types of wounds. The role includes case finding, consulting, making referrals, formulating treatment plans, educating the nursing staff and physician staff, auditing charts, collecting data and developing / managing quality improvement processes, developing / updating skin care policies and procedures. Reports to the Director of Nursing Quality and Professional Practice.
