SUMMARY Under the direction of a NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician-ordered occupational therapy services.

STATUS: Full Time LOCATION: Unity Hospital - 1555 Long Pond Rd, Rochester, NY 14626 DEPARTMENT: Physical Therapy SCHEDULE: Day shift, 8-hr. shifts, 8 am - 4:30 pm; weekend rotation requirement 1-2 weekend shifts per month RESPONSIBILITIES Within standards of practice established for a PTA, completes and documents (within agency standards) related to patient care, including visit notes, renewal orders, and verbal orders.
Includes progress or lack of goal achievement in all charting.
Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment.
Confers with therapist directly, physician, and/or interdisciplinary team members regarding patient response to therapy and any concerns.
Provides PT gait and ambulation training.
Instructs in the use of assistive devices.
Provides instruction in safe transfer using appropriate body mechanics and equipment.
Participates in discharge planning and provides timely and adequate patient and family teaching to facilitate the discharge plan.
Coordinates patient schedule with team members, readily and effectively adjusts schedule to meet changes in census and patient needs.
Maintains professional growth through active participation in continuing education programs, departmental in-services, etc.
REQUIRED QUALIFICATIONS Associate's Degree from an accredited Physical Therapy Assistant program Rehab Only: Successful completion of all fieldwork requirements and eligible to sit for the PTA or OTA exam.
Reads, writes, and speaks English Must demonstrate computer proficiency.
Excellent interpersonal and communication skills.
EDUCATION: AS: Physical Therapy (Required) LICENSES / CERTIFICATIONS: PTA - Physical Therapist Assistant certification - New York State Education Department (NYSED)New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

PAY RANGE: $22.60 - $32.00 CITY: Rochester POSTAL CODE: 14626 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
It may be modified in the future and eligible for additional pay components.
Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
