The Service Coordinator assesses client needs and develops personalized service plans to connect individuals with essential social assistance. They act as a liaison between clients and providers, advocating for resources and maintaining detailed case records.
A bachelor's degree in Social Work, Psychology, or a related field is required, along with at least 2 years of experience in case management. Proficiency in social service programs and the ability to maintain confidential records are essential.
Redwood Coast Regional Center (RCRC) is one of 21 Regional Centers that are community-based, private non-profit corporations under contract with the California Department of Developmental Services. RCRC provides intake, assessment, diagnosis, and coordinates community-based services for over 10,000 children and adults with developmental disabilities in Del Norte, Humboldt, Lake, and Mendocino counties. We work in partnership with many individuals, family members, community leaders, and agencies to help the people we serve live quality, independent lives in our region.
