The Social Worker plans and implements social work services for participants and caregivers, conducting psychosocial assessments and coordinating care with an interdisciplinary team. They act as a primary liaison for families, manage housing and facility transitions, and ensure compliance with CMS and state regulations.
Requires a Master's degree from a CSWE-accredited school of social work and a valid driver's license with personal transportation. Candidates must have at least one year of experience in a healthcare setting working with the frail or elderly, and specific licensure (LMSW/LCSW) is required for New Mexico.
