The Wellness Coach provides case management and supportive counseling to help consumers access community services and maintain daily living skills. Responsibilities include developing individual service plans, conducting home visits, and coordinating with therapeutic support services.
Requires a Bachelor's degree in Social Work or a related human service field with two years of experience in case management or working with vulnerable populations. Proficiency in Microsoft Office and bilingual skills in English and Spanish are preferred.
REQUIRED EDUCATION AND EXPERIENCE Bachelor’s degree in Social Work or a related human service degree with two years’ experience working with the people served and/or providing case management 2 years’ experience working with individuals with mental illness, substance use issues and/or a history of homelessness or 2 years’ experience working in the human services field required Experience in group work, tenant organization, case management, referral, and/or community outreach strongly preferred Experience or training in peer counseling or advocacy preferred Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System PREFERRED QUALIFICATIONS AND SKILLS Bilingual English/Spanish speaking preferred Company Overview S:US IS AN EQUAL OPPORTUNITY EMPLOYER Join a team of employees who care about the wellbeing of others.
