Perform case management and clinical activities for assigned patients within the Wound Care Center. Collaborate with physicians to evaluate and establish patient plans of care while participating in data conferences.
Requires a nursing degree, current Massachusetts RN licensure, and BLS certification with at least one year of related experience. A BSN and WOCN certification are preferred.
Under direction of the Manager, Wound Care Center or designee, perform a variety of duties involving case management of assigned patients including but not limited to wound care and other clinical activities associated with providing safe patient care. Responsible for periodic evaluation of assigned patient plan of care and in collaboration with Physician establish an alternate plan is needed. Participate in patient and wound care data conferences and continuing education as determined by Program Director and Manager.
Graduate of an accredited School of Nursing is required. Bachelor of Science in Nursing preferred. Wound, Ostomy, Continence Nurse Certification (WOCN) is also preferred. A minimum of one year related work experience required. Must demonstrate working knowledge of Microsoft Office (i.e. Word, Excel, PowerPoint, etc.) as well as related database programs. Current RN Licensure in the Commonwealth of Massachusetts is required. Basic Life Support (BLS) certification is required. Certifications in Medical Surgical, Home Health and Wound Care preferred. Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired Compensation: Pay rate will be determined based on level of experience. Pay Range USD $42.37 - USD $77.81 /Hr.
