The Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual’s health needs. They facilitate the progression of care and monitor the patient's progress to ensure high-quality, patient-focused services.
Candidates must be graduates of an accredited nursing school with a preference for a baccalaureate degree and have 3-5 years of hospital adult acute care nursing experience. Current RN licensure in Louisiana and BLS certification are required, along with effective communication skills and a basic understanding of healthcare regulations.
Graduate of accredited school of Nursing, Baccalaureate degree preferred. Current RN licensure by the Louisiana State Board of Nursing 3-5 years of hospital adult acute care nursing experience preferred. Current BLS certification through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: American Heart Association and American Red Cross. The ILCOR course must include a manikin check off. CCM, CMAC, or ACM preferred. Ability to demonstrate effective written and verbal communication skills and possess emotional and professional maturity. Basic understanding of Medicare, Medicaid, and Managed Care regulations. Organized and efficient in establishing priorities and performing tasks within specific timeframes. Self-motivated requiring minimal supervision. Views problems as a challenge and investigates alternatives which demand creativity, flexibility, and the ability to work under stress. Responsible for maintaining strict confidentiality of all verbal and written information and documents. Familiarity with and appropriate use of community resources. PHYSICAL DEMANDS: Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise.
