The ACT Case Manager is responsible for Person-Centered Planning to develop Individual Plans of Service. They provide ongoing linking, coordinating, and monitoring of services for individuals in the community.
Requires a bachelor's degree in a human services related field and one year of experience working with persons with mental illness. A valid driver's license and reliable transportation are mandatory for community travel.

We are seeking an ACT / IDDT Case Manager responsible for Person-Centered Planning to develop an Individual Plan of Service. The ACT Case Manager will provide on-going linking, coordinating and monitoring of services.
00 am to 5:00 pm, some evenings and weekend may be required. There are also rotating on-call shifts. This position is in-person at 1131 Ionia Ave NW, Grand Rapids, MI 49503. The person in this role will also travel within the community on a daily basis.
