Provide holistic, patient-centered palliative care and symptom management for individuals with serious illnesses in the Grand Rapids area. Lead goals-of-care conversations and coordinate care transitions between patients, families, and hospice teams.
Requires a current Michigan Nurse Practitioner license and DEA Certification for prescribing controlled substances. Candidates must have at least 2 years of experience in palliative, hospice, or home care.
