Provide advanced nursing practice and specialized primary care to patients and families in a home setting. Coordinate care for complex conditions and serve as a clinical resource for the interdisciplinary team.
Requires a current unrestricted NP or CNS license, national certification (ANCC/AANP), and 1+ years of experience in primary or chronic care. Must possess necessary prescribing credentials (DEA, NPI) and a valid driver's license.
As the Nurse Practitioner, you will be responsible for providing an advanced level of nursing practice to patients and families and a clinical resource to staff and the interdisciplinary team. $2,500 Sign-on Bonus for External Candidates
Delivers specialized primary care and consultation, including prescriptive authority in collaboration with a supervising physician Coordinates and manages care for home patients with complex conditions, including conducting comprehensive assessments and directing interdisciplinary team meetings Performs face-to-face patient visits at the home and ensures proper documentation using company-defined processes and forms Provides clinical education internally and externally, and serves as a resource for staff while participating in team meetings and quality improvement initiatives Supports performance improvement and fulfills additional duties as assigned, contributing to overall clinical excellence

Current unrestricted license in the state of residence Certified as a Clinical Nurse Specialist or Nurse Practitioner in the state of practice Current certification by a national body such as ANCC or AANP Possession of NPI Number, DEA with prescribing privileges, PECOS enrollment, CAQH enrollment, Medicare number, and referable Medicaid number Current CPR certification 1+ years of NP experience in chronic disease management, family care, or primary care Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation
