The Counselor provides direct counseling and case management services, including psychosocial assessments and individualized treatment plans. They are responsible for maintaining caseloads according to state regulations and preparing aftercare transition plans for patients.
Candidates must have at least an associate degree in chemical dependency, psychology, social work, counseling, nursing, or a related field. A CADC certification is preferred.
Maintain caseload per State Regulations Assures incoming patients have psychosocial assessment and a interpretive summary according to the timeline in the ARS Policies and Procedure Manual Completes, with patient participation, a Preliminary Treatment Plan according to the timeline in the ARS Policies and Procedure Manual. Completes a Treatment Plan Review and update for each patient after completion of the Comprehensive Treatment Plan according to the timeline in the ARS Policies and Procedure Manual Prepares an aftercare transition plan for all patients at the earliest possible time and to review prior to discharge Be proficient with State Licensure Standards, CARF Standards, 42 CFR and HIPAA standards and Company policies Qualifications A minimum of associates degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing, or a related field of study CADC preferred
