The therapist is responsible for conducting intake screenings, comprehensive assessments, and developing treatment plans for individuals with mental illness or substance use disorders. Candidates must hold a Master's degree in Social Work, Psychology, or Counseling and possess an active Illinois licensure (LPC, LSW, LCPC, or LCSW). Proficiency in telehealth services and evidence-based interventions is also required.

Determine and document diagnosis based on comprehensive assessment. Develop treatment plans in accordance with state regulatory/CARF guidelines. Participate in clinical staffing/treatment meetings and provide feedback on pertinent cases. Adhere to weekly service provision model. Work closely with the insurance department to ensure authorized services are approved and adequate information is provided within specified time frames. Education and/or Experience Qualifications: Master’s degree in Social Work, Psychology, Counseling, or a related field required. Must hold active Illinois licensure as LPC, LSW, LCPC, or LCSW. Must be comfortable providing telehealth services.
