Hospice Field LVN/LPN

• The Hospice Field LVN/LPN plays a critical role in providing compassionate, patient-centered care to individuals facing life-limiting illnesses in their homes or care facilities.
• This position focuses on managing and alleviating symptoms, supporting patients' comfort, and enhancing their quality of life during end-of-life care.
• The LVN/LPN collaborates closely with interdisciplinary hospice teams, including physicians, social workers, and chaplains, to develop and implement individualized care plans.
• Additionally, the role involves educating and supporting patients' families and caregivers, ensuring they are equipped to provide appropriate care and emotional support.
• Ultimately, the Hospice Field LVN/LPN serves as a vital advocate for patients' dignity and comfort throughout their hospice journey.

As a Hospice LPN, you will

• Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs as evidenced by documentation, clinical record, team report, and evaluations.
• Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
• Applies concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Assure the continuity of care through delivery of quality patient care.
• Provide effective communication to patient/family, team members, and other health care professionals as evidenced by clinical notes, case conferences, communication notes, and evaluations.
• Complete appropriate clinical and progress notes, documenting outcomes of interventions.
• Assess patient/family response to care, ensuring communication of information to appropriate team members.

Requirements

• Current LVN/LPN License
• Valid Driver’s License
• Valid Auto Insurance
• CPR Certification
• HomeCare HomeBase experience preferred
