The specialist assists Care Facilitators and Social Workers in promoting timely and safe patient discharges. They coordinate essential services including home health, DME, and rehab placements.
A high school diploma is required, while a junior college or technical school degree is preferred. Candidates must have BLS certification and a working knowledge of medical terminology.
St. Mary's Medical Center is seeking a full time Discharge Planning Specialist. The Discharge Planning Specialist (DPS) will assist the Care Facilitator and or Social Worker with the discharge planning responsibilities, to promote timely and safe discharge services. Under the supervision of the Care Facilitator and or Social Worker, the Discharge Planning Specialist facilitates and coordinates discharge services such as home health, DME, Rehab placement, and other community resources or agencies.
High School Diploma is required. Junior College/Technical School preferred.
Education and training in hospital healthcare transitional care, and discharge planning beyond the hospital setting, preferred.
BLS required. Working knowledge of medical terminology. Excellent communication skills-verbal and written ability to work with patients, families, physicians, and other hospital departments.
