The Housing Wellness Coordinator provides on-site behavioral health and social services to seniors in supportive housing to help them age in place with dignity. Responsibilities include conducting assessments, implementing wellness activities, and providing supportive counseling under clinical supervision.
Requires a Bachelor's degree in human services, psychology, or a related field and two years of experience with elderly or mental health populations. Proficiency in English and Chinese, Korean, or Spanish is required.
Conduct and prepare assessments, case logs, documents case notes, care plans for tenants. Establish and maintain contacts with families, social service support agencies, and health facilities. Assesses residents for their needs and develops and implements service plans. Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services. Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goals. Develops wellness goals and activities with the input of residents and other staff. Ensure the development of appropriate wellness, educational and social programming is consistently taking place. Coordinates care for targeted critical/high need resident cases. Assist Licensed Practical Nurse with medication management. Work through team approach in partnership with building property management and maintenance staff to identify and respond to residents in need. Respond to emergency situations on site and provide crisis intervention. Maintain contact and follow-up with service providers put in place for residents such as Adult Protective Services and/or outpatient treatment services. Strictly adhere to and enforce all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintain compliance with HIPAA regulations. Accompany residents to face to face meetings as requested by the resident. Attend staff meetings, agency meetings, workshops, classes, & in-service trainings on a regular basis to maintain current information on eligibility requirements for all basic entitlements.
Bachelor’s degree in human services, Sociology, Psychology, Social Work or related field Two years’ experience working in housing, mental health, senior and/or elderly populations or related fields. Excellent organizational, verbal, & written communication skills. Ability to work independently as well as part of a team. Bi-Lingual Chinese/Korean/Spanish/English required. Regularly required to talk, hear, walk, stand, & sit. Able to operate a computer keyboard, mouse & office equipment. Ability to work flexible hours (evenings/weekends) as assigned. Able to travel throughout the five boroughs as needed. Ability to lift up to 10lbs.
