The Case Manager provides comprehensive case management to clients in a family shelter, including intake and the design of social service plans. They advocate for families to secure entitlements and assist them in obtaining permanent housing.
A Bachelor's Degree is required along with previous experience working with the homeless population. Candidates must possess exceptional oral and written communication skills and the ability to manage a diverse caseload.
Completes family intake, comprehensive family history, and maintains accurate and timely family case records. Responsible for designing social service plans in cooperation with new residents. Set and follow-up on long and short-term client goals and objectives. Secures entitlements for clients and serves as an advocate for families in conflict. Responsible for advocacy and providing assistance to clients in obtaining permanent housing. Make referrals to special housing agencies. Meet with clients to develop strategies to meet their housing needs. Develops an individual program of housing readiness, parenting skills, prevocational and vocational counseling as necessary. Works collaboratively with family monitors. Participate in and attend the Interdisciplinary Team and Administrative meetings weekly. Submit all monthly reporting in a timely manner.
