$44 - $60 / HOUR
Provides counseling and support to patients and families to manage needs incidental to hospitalization and transition planning. Facilitates discharge plans, coordinates hospice care, and develops individualized plans of care based on psychosocial assessments.
Requires a Master's Degree in Social Work, specifically for candidates in California and Hawaii. Preference is given to candidates with one to two years of experience in acute or long-term medical settings.
Provides services to patients and their families to deal with needs incidental to their hospitalization. Provides counseling, support and assistance in making transition plans. Provides direction for routine aspects of non-medical problems of patients and their families. Assists with the planning and post-discharge of patients with outside agencies, nursing staff, and relatives.
