• What you’ll do
• Perform comprehensive patient evaluations and develop individualized treatment plans
• Deliver hands-on, goal-driven physical therapy to improve mobility, strength, and function
• Track patient progress and adjust care plans to maximize outcomes
• Educate patients and families to support recovery and long-term success
• Maintain timely and accurate clinical documentation
• Collaborate with healthcare team members to ensure coordinated, high-quality care

• What we’re looking for
• Licensed Physical Therapist (PT) in good standing
• Strong clinical skills with a patient-first mindset
• Excellent communication and documentation abilities
• Ability to work independently while contributing to a collaborative team
• Commitment to quality care and positive patient outcomes

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

• Physical Demands
• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Must be able to lift, carry, pull, or push up to 100 lbs..
