The Hospice RN serves as the primary point of contact for patients and families, delivering skilled nursing care and managing individualized plans of care. Responsibilities include performing assessments, collaborating with an interdisciplinary team, and providing emotional and spiritual support during end-of-life care.
Candidates must hold an active RN license, CPR certification, a valid driver's license with auto insurance, and have at least one year of experience in hospice, home health, or related fields. A compassionate, patient-centered approach and the ability to work in diverse home and facility settings are required.
