The Specialty Case Manager provides community-based case management, planning, and advocacy for high-risk individuals with mental health or substance use diagnoses. They are responsible for developing individualized treatment plans and connecting patients to essential health, housing, and social services.
A high school diploma or GED is required, with an associate degree in social sciences and 1-3 years of experience with special populations preferred. Candidates must possess a valid AZ Driver's License, fingerprint clearance, and preferably a peer certification.
