The Housing Liaison implements Housing First and Harm Reduction models to assist individuals experiencing homelessness in finding and maintaining stable housing. Key duties include conducting community outreach, recruiting landlords, and coordinating with the Continuum of Care for case conferencing.
Candidates must have a Bachelor's degree in Social Work, Psychology, Sociology, or a related field. Alternatively, three years of relevant case management experience in housing is acceptable.
The Housing Liaison Practices Housing First and Harm reduction models to improve outcomes and decrease barriers for individuals experiencing housing problems.
1. Attend local Continuum of Care meetings related to coordinated entry/case conferencing.
2. Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness
3. Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits.
4. Landlord recruitment and rapport building. Directly contact potential landlords and provide information and make referrals to their units. Follow up with landlords about referrals and problem solve tenant issues to maintain rapport.
5. Receive referrals from Continuum of Care programs to assist individuals with housing stability.
6. SOAR certification and assist individuals throughout the SOAR process.
7. Assist with the moving on process and meet with tenants to evaluate their readiness to move on to other permanent housing options.
Bachelor's in Social Work, Psychology, Sociology, or other related field and/or three years of relevant case management experience in housing.
