• The Discharge Planner provides individualized assessment and case management, supportive counseling, and discharge planning for patients and their families with mental health related problems utilizing problem-solving methods to restore or enhance social functioning.

Qualifications

Education

Bachelor’s Degree in social work or related field.

Experience

Experience in inpatient or outpatient mental health setting preferred.
