This role is essential in delivering compassionate end-of-life care, coordinating care plans, and assisting in urgent situations. The Hospice RN will assess patients’ needs, prioritize care, and offer clinical guidance over the phone. This position also supports Home Health and Home Care, and requires an RN with excellent communication skills, critical thinking abilities, and a deep understanding of hospice principles.
Conduct thorough triage assessments of patient symptoms, needs, and care requirements. Provide symptom management guidance, support, and education to patients, families, and caregivers. Coordinate with on-call hospice team members, including nurses, physicians, and other healthcare professionals, to facilitate timely care. Ensure appropriate follow-up care and documentation, keeping detailed and accurate records in the patient care system. Offer emotional support and resources to patients and families, guiding them through the end-of-life care process. Evaluate the urgency of care needs and escalate cases when appropriate. Communicate effectively with interdisciplinary teams to address patient needs and update care plans.
Current RN license and good standing with the state nursing board. Minimum of 2 years of hospice, palliative care, or acute care experience, with prior triage experience preferred. HomeCare HomeBase experience is a plus! Strong communication, problem-solving, and critical thinking skills. Proficiency in electronic medical record (EMR) systems. Ability to work independently, with empathy and professionalism, in high-stress and emotionally sensitive situations.
